JOB OPENING
MN Aging Pathways Resource Coordinator
Dancing Sky Area Agency on Aging is seeking a full-time 1 FTE Minnesota Aging Pathways Resource Coordinator. This position will be responsible for planning intensive care coordination for people wanting to remain in their home or relocate to the community after a stay in a care facility. Minimum of a bachelor’s degree from an accredited college or university in Nursing, Social work, Gerontology, or a related human services field. Experience working in person-centered discharge planning, charting, care coordination, or related tasks. Proficient in problem-solving skills, adept at phone etiquette, and strong in computer skills. This position is work from home, preferably in Ottertail, Wilkin, Traverse, Grant, Stevens, or Pope counties. Regular travel required, including throughout the 21-county service area.
Salary Range
$54,233 - $65,000 + Benefits
