The Northwest Regional Development Commission in Warren, MN is looking for a highly-motivated person to perform numerous general office, computer and receptionist duties.

Community Living Specialist (Part-time)
The Land of the Dancing Sky Area Agency on Aging, a program within the Northwest Regional Development Commission, is seeking a part-time (approximately 24 hours/week) MinnesotaHelp Network™ Community Living Specialist.
This position will be responsible for providing intensive care coordination and relocation assistance for nursing home residents to relocate from a long-term care setting back into the community.
Qualified candidates must have a minimum of a bachelor's degree from an accredited college or university in nursing (RN), social work, gerontology or another related human services field. Experience in working with nursing home or hospital discharge planning, public and privately funded programs and person-centered planning preferred. Candidate must have proficient computer skills.
The position entails working remotely from Clay County with some travel to surrounding counties.
Salary range $20.98-$24.56 per hour plus benefits package, including health insurance.
